VA Flags EHR Modernization Implementation Risks

VA’s Office of Inspector General said in a Jan. 23, 2026 annual report that the department’s electronic health record modernization remains a major management concern as VA prepares to resume Oracle Health EHR deployments in April 2026. The report cited 360 outages and other issues at initial sites, noted 32 OIG recommendations still open, and warned continued incidents could delay implementation and risk patient safety.
